Joshua DR. Manalo, MD
East Avenue Hospital Department of Orthopaedic Surgery
Philippines | Trauma Reconstruction Fellowship | January to June 2025

I’m very thankful for the opportunity to train for six months at the Trauma Reconstruction Center of Shinyurigaoka General Hospital under the guidance of Dr. Sawaguchi. This fellowship has been one of the most meaningful experiences in my career so far, not only for what I learned but also for how I grew as a surgeon and as a person.

From the first day, I was made to feel part of the team. I joined daily planning meetings, observed and assisted in surgeries, and helped follow up with patients. The cases were diverse, including pelvic and acetabular fractures, periarticular injuries, open tibial fractures, and more. I was exposed to both routine and complex cases, which gave me valuable experience and built my confidence.

What impressed me the most was the emphasis on careful preoperative planning. Every step was performed with precision, always considering the preservation of soft-tissue structures and the accuracy of bone reduction while ensuring strong, stable fixation. I saw how proper planning, imaging, and reduction techniques made a huge difference, especially in challenging cases. These principles will definitely guide my future practice.

I’m also very grateful to the junior doctors: Dr. Sano, Dr. Nakagawa, Dr. Toyonaga, Dr. Sato, Dr. Hamaguchi, Dr. Kameyama, Dr. Yamaoka, Dr. Imamura, Dr. Aoki, Dr. Shibata, and Dr. Yasutake, who were always approachable and willing to teach. Even with some language barriers, they took the time to explain, answer my questions, and involve me in case discussions. Their openness and support made a big impact on me.

I would also like to sincerely thank the senior doctors: Dr. Matsushita, Dr. Sawaguchi, Dr. Minehara, Dr, Kudo, Dr. Okazaki, and Dr. Takamure for their guidance, teaching, and encouragement throughout my fellowship. I learned so much by observing their techniques and listening to their insights during surgeries and conferences. Their experience and leadership helped shape my understanding of thoughtful, high-level trauma care.

This fellowship also gave me a firsthand look at how the Japanese healthcare system works. I was impressed by the strong interdepartmental collaboration, the respectful and thoughtful way patients were treated, and the overall efficiency of the hospital system. These are values I will carry with me and try to apply back home.

Outside the hospital, I enjoyed learning about Japanese culture, making new friends, and experiencing day-to-day life in Japan. Even simple moments like sharing dinner with the team after surgery made me feel more connected and welcomed.

As I leave Shinyurigaoka, I feel very fortunate to have trained here. I learned not only surgical techniques but also important lessons in discipline, teamwork, and professionalism. I will carry these lessons with me always, and I hope to return someday not just as a visitor but as a colleague and a friend.

To Dr. Sawaguchi and everyone at the Trauma Reconstruction Center, thank you very much for your kindness, generosity, and support. I will never forget my time here.

ありがとうございました。
